Community Connections – Te Hāpori Āwhina Tāngata, is a community-based service which provides services for young people and adults with disabilities, and their families.  They operate in the Greater Wellington region (Wellington, Hutt Valley, Porirua and Kapiti), Horowhenua, Manawatū, Taranaki, the Bay of Plenty, and the Hawke’s Bay area. They have a strong commitment to a person-centred approach so people can live the life they choose within their own community.

They offer these services:

  • Supported Living supports you to live in your own house.
  • Residential Support means you receive support whilst living in a house rented by Community Connections.
  • Neighbourhood Connections brings together groups of people who use their skills and talents to help each other, while forming lasting friendships.
  • Employment Connections provides employment support and transition services.
  • ACC Living My Life supports people with an acquired injury.
